Get Medical Records

Obtain Medical Records (Optional)

We recommend (but do not require) bringing the last two years of medical records, lab tests, and EKG’s to the initial appointment for patients with extensive or complex medical histories. We can also request records on your behalf.

This step is optional, but it can help us start a treatment plan sooner in some cases.

How to Get Your Medical Records

  1. Login to your medical provider’s patient portal and print your records.
  2. Request medical records from your doctor for your own use.
  3. To send us your records,  Fill a Release Form.
(Records should contain two years of labs and EKG’s.)

Your First Consultation

During your initial visit, we will:

  • Examine your medical history and lab results.
  • If a recent EKG is not available, perform one.
  • Conduct a brief physical examination.
  • Discuss and implement treatment options and other recommendations.
  • Respond to questions and concerns.
  • Make a follow-up appointment.

Follow-Up Appointments

Regular appointments keep you on track to achieve your goals! We intend to:

  • Examine changes to your medical history, labs, and vitals.
  • Discuss the current treatment plan and its effectiveness, as well as whether other options are appropriate.
  • Discuss diet and exercise recommendations when appropriate
  • Respond to questions and concerns.
