• • • Our practice has established appointment policies to ensure efficient and effective care for all patients. These policies include fees for cancellations, no-shows, and late arrivals.
    • Please provide us with at least 48 hours’ notice if you need to cancel or reschedule your appointment. Otherwise, we will charge you a $25 Appointment Cancellation Fee. If you fail to cancel your appointment in advance and do not show up, we will charge you a $40 No-Show Fee. You must pay these fees in full before scheduling another appointment.
    • If you arrive late to your appointment, we may ask you to reschedule. If you arrive very late and cannot be seen by a provider, we will charge you a $40 Late Fee.
    • For Saturday appointments, you must prepay, and this payment is non-refundable. If you need to cancel or reschedule your Saturday appointment, please provide us with at least 48 hours’ notice. Otherwise, you may forfeit the prepayment.
